With the favour of house Beaumont adorned appropriately, Simon de Montfort stepped forward to the field.
He offered his opponent a bow before the match started, and would hear him out before they came to blows.

John Castle wears the favor of Isabel de Warenne. He raised his visor to show the Frenchman the proper respect, but it seemed he didn't have many words for him. Just an acknowledgment and a granite-set game-face.
Trotting to the end of the list on his charger, he dips his lance to you and then sets to charge.

1d20 15 This is, unfortunately, a critical success. Unless you succeed with a roll above a 15, you will suffer real damage as the tip slides into your underarm and tastes flesh.

The excess above 20 does indeed get added to the roll, so you did achieve the critical success, inflicting 19 true damage. While this is not enough to knock Castle off of his horse outright, because we are dealing with true damage, we have to look at Unconsciousness and Wounds. Castle's Total Hit Points are 28. His Unconsciousness threshold is 7, so he does not fall unconscious.
Major Wound: Damage that is equal or greater than CON (which in Castle's case, 14, this does), inflicts a Major Wound. That's Bad.
1. The “Chirurgery Needed” box is checked immediately. The character requires the attention of an expert in order to avoid infection and other dangers resulting from his injury, described as deterioration in these rules.
2. The character falls unconscious unless he makes a successful d20 roll against his current hit points (for example, a character with 10 hit points left would need to roll a 10 or less to stay conscious).
3. The player must roll on Table 5–3: Statistics Lost to discover the permanent effect, if any, of the wound. All derived statistics, such as Movement Rate or Unconscious, must be refigured immediately if a loss to an attribute occurs from this roll. If the Total Hit Points statistic drops, current hit points are not affected unless the character is at full hit points currently (in which case they drop to the new maximum).
4. Should the character avoid unconsciousness, he may wish to continue fighting or performing some similarly active task. However, the injured character must first make a successful Valorous roll to do so, possibly with a modifier. Failure indicates the knight cannot summon up the courage and desire to continue fighting. He hesitates, and will not enter combat unless forced into it. A fumbled Valorous roll means the knight actually flees in fear or surrenders. A critical success may gain some benefit, at the Gamemaster’s discretion.
5. If further actions are taken once a Major Wound is suffered, both aggravation and deterioration may set in.

Castle falls unconscious from the anguish of catching a lance in such a way as to inflict excruciating anguish.

Castle will live with the memory of the sapping agony of being transfixed like a worm, wriggling on a hook, for the rest of his life.

Castle falls from his horse with the force of the blow.

He takes 4 damage from the impact with the muddy ground.

He is now unconscious, lying in the muck, a sucking wound in his side from the lance of Simon de Montfort.

Seeing his opponent fall, he recognized his victory - however, he is no stranger to how close to death he was. As he crashed into the Castle of a man he felt his opponent's lance trace the underside of his arm before he loses the strength to wield it.
Holding up his token in victory, he rode to his opponent's side to offer the help and congratulate them.
"Well fought, sir. You almost ha-sir?"
He lightly shook the knight for response.
"Hold your head high, you have done w- Sir? Sir?!"
He shook him more intensely - to no response.
Realizing his error, he drew upon his inner anguish and guilt to unleash a howl of urgency, one which caused his very ribs disturbance. With horror in his voice, he cried out to the world but one word:
"MEDIC!"

Margaret continues down from the bleachers, her small for, deftly threading the crowd like a thread does the eye of the needle. Rushing across the lists, she begins assisting the young knight. With quick fingers, she deftly undoes the knots around the knight, knowing as she pulls the plates away that she’ll spend long hours tending to this man later. Still, it will do Simon’s heart good to see him live, so she does as she can.
She can hear Miriam coming up behind her, so she does what she can to staunch the bleeding as she makes space. They're both skilled at sealing wounds, though she's better at long-term while Miriam is better at short term. So she'll focus on stopping the bleeding and letting Miriam get on with it while she readies herself for chiurgery.

Seeing the arrival of others, he tries to focus. He thinks through his bible study, reciting the verses as he remembers them.
Though shalt not kill.
He begins moving what he can, slipping off the helmet as his hands shake.
And if a man have committed a sin worthy of death, and he be to be put to death, and thou hang him on a tree:
He looks to Margaret, trying to force his mouth to speak, but the noose within his own guilt constricts him, strangling him of breath.
Have the gates of death been opened unto thee? Or hast thou seen the doors of the shadow of death?

Giles comes dashing onto the field as fast as his little legs will carry him, the sucking mud pulling at him as he struggles through its thickness.
Margaret beat him there, but he would help however he could the pair working well together to do what could be done for knight Castle on the field.
:He can't stay in these conditions for long, as soon as we plug up this hole, we need to get him out of this muck."

Roll 1d3 to restore the hp from First Aid.
He will still need a Chirurgery roll to have the Major Wound not grow worse.

The initial treatment was moderately effective, but there were too many foreign particles getting into the wound, risk of infection was high.
He glanced to Miriam as he assisted the foreigner, this did not look good.

Margaret elbows the others aside. "I'll fix him, you all go about your business. Maybe some of you strapping young lads could help me carry him elsewhere. Simon, grab his armor and bring it with you. Miriam, follow me."

Margaret was right. There were too many people around, too many hands in the young man's guts.
He stepped back letting them move Knight Castle to the infirmary while he maintained vigil over the field against any further catastrophe.
